Thoracic Epidural Anaesthesia Was Not Able to Attenuate Stress-Induced Immunosuppression in Patients Undergoing Major Abdominal Surgery

چکیده
Background: Blunting the perioperative stress response and preserving immune function are critical issues for patients undergoing major abdominal surgery. Aim: The present study investigated whether perioperative thoracic epidural block affects postoperative immune response in patients undergoing major abdominal surgery. Patients and Methods: Patients undergoing major abdominal surgery were randomly assigned to either general anesthesia with continuous epidural infusion via epidural catheters that was continued for postoperative analgesia (group E, n=15) or intraoperative general anesthesia and postoperative IV morphine analgesia (group G, n=15). Plasma levels of Creactive protein (CRP), leukocyte counts, and percentage of total lymphocytes were assessed before and after surgery and on postoperative days (PODs) 1 and 3. Results: There were no significant differences in variables between groups. In both groups, Leukocyte counts were increased on POD1 (each group p<0.05) and POD3 (each group p<0.01). The proportion of lymphocytes decreased from the end of surgery to POD3 (each group p<0.01) and CRP were still increased on POD1 and POD3 (each change, each group p<0.01). Conclusion: Tissue damage and inflammation apparently overcome the effects of thoracic epidural block on stressinduced immunosuppression in patients undergoing major abdominal surgery.

Thoracic Epidural Anaesthesia in Awake Upper Abdominal Surgery: Safety/Validity of Bromage’s Formula
Objective: Subtotal gastrectomy has been successfully performed using thoracic epidural anaesthesia alone in two high-risk surgical patients. We describe these two cases by using the Bromage’s formula to calculate the required doses for a satisfactory segmental block. Case Report: Two high-risk surgical patients were scheduled for sub-total gastrectomy. They received thoracic epidural anaesthes...
متن کاملEffective awake thoracic epidural anesthetic for major abdominal surgery in two high-risk patients with severe pulmonary disease--a case report.
Awake thoracic epidural anaesthesia as the sole anaesthetic technique was successfully employed for two high risk surgical patients with chronic obstructive pulmonary disease undergoing abdominal surgery. The procedure was tolerated well and the anaesthetic technique which has been shown to reduce intra-operative and post-operative cardiac, respiratory and gastrointestinal complications, may ha...
متن کاملCombined Epidural-General Anesthesia (CEGA) In Patients Undergoing Pancreatic Surgery: Comparison Between Bupivacaine 0.125% And 0.25%
Background: Major abdominal surgery results in homodynamic instability mainly due to endogenous prostacyclin release, bleeding, major intercompartemental fluid shift and the hormonal surgical response. This study compared the effects of low thoracic epidural anesthesia with 0.125% and 0.25% bupivacaine on haemodynamic variables, sevoflurane requirements, and stress hormone responses during panc...
متن کاملWhipple’s Operation Under Lumber Epidural Analgesia and Sedation
Whipple’s operation can be carried out either under general anaesthesia or continuous epidural anaesthesia with sedation. We report three cases that were managed successfully and we found that the combination of lumbar epidural analgesia with sedation is safer and beneficial as compared to those who undergo Whipple’s operation under general anaesthesia. For the last three decades epidural analg...
متن کاملNon-analgesic effects of thoracic epidural anesthesia.
Thoracic epidural anaesthesia is selected usually to provide adequate postoperative analgesia; however with administration of local anaesthetics to epidural space selective sympatolysis ensues. The effects of this transient sympathectomy on cardivascular, respiratory and other systems deserve certainly some interest as it may influence postoperative morbidity or mortality. Thoracic epidural ana...
